사카구치 시몬이 1995년 발견한 CD25+ T세포는 약 10년에 걸친 체계적 연구의 결실이었다. 사카구치는 1985년 <Journal of Experimental Medicine>에 발표한 논문에서 정상 마우스의 특정 T세포 서브타입을 제거하면 자가면역질환이 유발됨을 관찰했다. 이는 자기 세포와 반응해 자가면역반응을 일으키는 T세포가 존재하며, 이것이 다른 T세포 집단에 의해 억제된다는 첫 번째 증거였다. 그러나 이렇게 면역 기능을 조절하는 T세포가 정확히 어떤 특성을 가지는지, 이들을 구분할 수 있는 바이오마커가 무엇인지는 당시 알려지지 않았다.

1989년 사카구치는 갓 태어난 마우스에게 면역억제제인 사이클로스포린(cyclosporine)을 투여하면 활성화된 T세포가 선택적으로 제거되며, 이후 성체가 된 시점에서 약물을 중단하면 자가면역질환이 발생함을 확인했다. 이는 정상적으로 활성화된 특정 T세포가 면역 관용에 필수적임을 시사하는 것이었다. 1990년에는 흉선에서 자가반응성 T세포와 조절 T세포가 모두 생성됨을 밝혔다. 이는 자가면역질환의 발생이 자가면역을 일으키는 T세포뿐만 아니라 조절 T세포와의 균형 상태에 의해 결정됨을 의미했다.
