소수의 환자 살리는 신약개발
제약산업이 '희망산업'인 까닭

필자의 회사에서는 지난 6월 정 모 어린이(8)의 치료비를 마련하기 위해 임직원 바자를 열었다. 태어나자마자 친부모에게 버림받아 시설에 맡겨진 정양은 양부모에게 입양됐으나, 돌 즈음 희귀질환인 고셰병 확진을 받았다. 현재 병세가 악화돼 특별한 보살핌이 절실하나 경제적 어려움을 겪는 양부모 밑에서 수차례 파양될 위기를 겪고 있다. 겨우 8살 아이가 겪기엔 너무 큰 아픔이 아닐 수 없다.

고셰병은 국내 환자 수가 40명 정도밖에 되지 않는 희귀질환이다. 대부분의 환자들이 적절한 치료를 받는다면 정상적인 생활이 가능하나, 병을 치료할 방도가 없던 과거에는 성인이 되기 전에 사망할 수밖에 없었다. 우리 회사의 희귀질환 사업부인 젠자임의 설립자들이 고셰병 치료를 위한 효소치료제 개발을 시작한 것은 1980년대 초로 거슬러 올라간다. 이들은 트렁크에 아이스박스가 실린 차를 직접 몰아 미국 북동부 지역의 모든 병원을 돌며 임상에 필요한 효소를 추출하기 위해 태반을 모았다. 당시 고셰병으로 진단된 12명의 환자에게 희망을 선물하기 위한 힘겨운 여정의 일환이었다. 결국 이 같은 노력 끝에 브라이언 버먼이란 환자가 세계 최초로 고셰병 치료에 성공했고, 그는 결혼 후 아이를 낳고 정상적인 생활을 영위하고 있다.

비록 소수일지라도 하나의 생명에게 희망을 선물한다는 것은 가늠할 수 없는 의미를 갖는다. 2010년 우리는 지하 700ｍ의 갱도에 갇혀 있던 33명의 칠레 광부들의 구조 과정을 애타게 지켜보며 ‘기적의 생환’을 기도했다. 나와는 관계 없는 소수의 사람들이지만 위험에 처한 생명을 위해 희망과 기적을 바라는 모습을 지켜보며, ‘한 사람이 지닌 생명의 가치와 희망을 추구할 권리’를 되새겨 보게 되었다.

소수의 희귀질환 환자들의 완치를 위한 관심과 노력은 맞춤형 의학 시대를 열어가는 데 도움이 되고 있다. 이제 ‘유전자염기서열 분석’이라는 기술을 통해 환자 개인의 맞춤형 진단 및 치료, 예방이 가능한 시대가 오고 있다. 희귀질환을 앓고 있는 환자들도 개인에 특화된 진단 및 치료를 받을 수 있게 될 것이다.

희귀질환을 가진 두 딸의 투병기를 쓴 프랑스 여성 안-도핀 줄리앙은 “누군가의 삶에 시간을 더할 수 없다면, 그 사람의 남은 시간 동안 진정한 삶을 선물해야 한다”고 말했다. 비록 소수일지라도 우리는 환자들의 남은 삶을, 그리고 그 삶을 채워나갈 희망을 외면해서는 안 된다. 세계보건기구에 등록된 5000여종의 희귀질환 중 치료제가 개발된 질환은 300여개에 불과하다. 비록 소수의 환자일지라도 끊임없는 연구·개발과 혁신을 추구하는 노력이야말로 바이오 제약산업이 진정한 의미의 ‘희망의 산업’이 될 수 있는 이유가 된다.

The Value of Hope

Our company held a bazaar to raise a fund for treating a child patient named Jeong Hyeon-ji (8 years old, alias) in June. Hyeon-ji was adopted by foster parents after she was abandoned by her own parents upon birth and protected by a child care facility, but she was diagnosed with Gaucher’s disease, a rare disease, around her first birthday. She desperately needs special care as her disease is aggravated, but she has been experiencing several crises of losing the adoptive relation because her foster parents are undergoing financial difficulty. It is a really torment and pain too great to be endured by an 8-year old child.

The Gaucher’s disease is a rare disease with only about 40 patients in Korea. Most patients can lead a normal life if they are properly treated, but in the past when no therapy was available, such patients had to die before they were grown up to adults. The start of development of an enzyme treatment for Gaucher’s disease by founders of Genzyme, the rare disease division of Sanofi Group, dates back to early 1980s. In early period, they visited all hospitals in northeastern part of the United States by driving vehicles with ice boxes in the trunk by themselves to collect placentas for extracting enzyme needed for clinical treatment. It was a part of difficult journey to present hope to 12 patients who were diagnosed with Gaucher’s disease then. As a result of such strenuous efforts, a patient named ‘Brian Berman’ succeeded in treating Gaucher’s disease for the first time in the world. And he is now leading a normal life after getting married and giving birth to his baby.

Though they are small in number, presentation of hope to a life has an unfathomably great meaning. In 2010, we prayed for the ‘miracle of safe return’ of 33 mine workers in Chile who were locked in 700 meter-deep mine shafts under the ground while watching their rescue processes. Although they are small number of people who have no relationship with me, I had a chance of meditating the ‘value of life and right to pursue hope of a person’ while I was watching people wishing for hope and miracle for people who were facing danger.

People’s interest and effort for complete cure of small number of patients with rare diseases are helpful for opening the customized therapy era. With development of the ‘gene sequencing’ technology, the era is approaching when individual patients can receive customized diagnosis, treatment and prevention. Patients who are suffering from rare diseases like Hyeon-ji will be able to receive individualized diagnosis and treatment.

A French woman named ‘An-Dauphin Julian’ who wrote stories on struggles against rare diseases of her two daughters said, “When you cannot add times to someone’s life, you can add life to that person’s time.” Though they are small in number, we should not disregard remained life of patients and the hope that can fill their remained life. Among about 5,000 rare diseases registered at the WHO, treatments for only around 300 diseases are developed. Even though they may be made for small number of patients, incessant efforts for R&D and innovation will be reasons for developing bio-pharmaceutical industry as a ‘hope industry’ in its real sense.
